Evaluation of leukocyte esterase reagent strip test for rapid bedside diagnosis of spontaneous bacterial peritonitis

Spontaneous bacterial peritonitis (SBP) is a common complication of cirrhosis with high morbidity and mortality. Early diagnosis treatment improve survival. Diagnosis made by ascitic fluid Polymorphoneuclear leukocytes (PMNL) count >250/mm3 which takes hours may not be available in rural settings. Leukocyte esterase reagent strips (LERS) test have shown sensitivity, specificity, negative predictive value the SBP. This study was conducted to find utility LERS for prospective hospital-based at National Academy Medical Sciences, Bir Hospital, Nepal. performed on from 140 cirrhotic patients. Colorimetric grading compared PMNL diagnosing Sensitivity, positive (PPV), (NPV), accuracy SBP calculated grade 3 4 as cut-offs. Among patients, diagnosed 27. Grade cut-off, PPV, NPV were 96.3, 90.2, 70.3, 99 percent respectively. For 81.5, 99.1, 95.6, 95.7 Accuracy 91.4 95.2 cut-off has ascitis. Being simple, rapid, cost-effective, it can useful bedside start early antibiotic therapy before availability report.
